Team Staff Evaluations Available
HMHA would like to hear feedback from parents and players on the 2017-2018 season by completing a HMHA Team Staff Evaluation survey so we can plan and make improvements in the future. HMHA would like to strongly recommend that all players submit a survey so we are able to gather more accurate information on each team. Please remember all HMHA team staff and board members are volunteers.
Managers on REP/AE teams were provided with team staff evaluations along with a return envelope. Please take this opportunity to provide some constructive feedback on team staff and comments for HMHA. If you would like a response back from a HMHA board member, please ensure to include your name and contact info.
These forms will also be available to all Local League parents/players through their convenors of their divisions as well.
Anyone who is unable to obtain a survey from a manager or convenor, can contact Shannon Lockwood at [email protected] and one will be forwarded to you to complete. Please return completed form to your manager or deposit in the ballot box located in Oliver's.
