Huntsville Minor Hockey Association (HMHA) and Ontario Minor Hockey Association (OMHA) requires ALL Rostered Volunteers to complete a Vulnerable Sector Check (VSC) with the OPP. For more information regarding VSCs please review the OMHA Criminal Record Check Policy.pdf

3 Steps to Complete a NEW Vulnerable Sector Check (VSC)
Step 1: a) Click VSC Volunteer Letter 2022-23.docx. to download word document b) Fill out Volunteer Letter c) Save locally as PDF/JPEG to your computer
You will be required to attach this letter when prompted to do so during the electronic identity verification process (step 2)
Step 2: a) Click Electronic Identity Verification (EIV). b) Scroll to bottom to "Apply for a Record Check"
(Please review OPP steps prior to applying for record check)
Step 3: Email your completed VSC (not the confirmation email) and state Hockey Division & Role to [email protected]

VSC HMHA Declaration Letter
You must complete (Click to download/complete) HMHA Declaration Letter(Fillable PDF) ONLY if you have a current (not expired) Vulnerable Sector Check (VSC) on file with the association. This letter confirms that since your last VSC you have not been convicted of any criminal offence.
The declaration is not to be sent to the OPP, simply save as a PDF/JPEG and email Letter to [email protected]
